SANDY, Utah (AP) - An independent investigation has determined a Salt Lake City suburb violated technical notice rules and should have warned residents sooner when part of the city’s water system became contaminated.

The Salt Lake Tribune reports a law firm examined the city of Sandy’s response when a pump failed tainting drinking water with an excess of fluoride in early February.

Officials say fluoride is beneficial in small amounts but too much can cause health problems.

The report released Thursday faulted the city with how it communicated the problem to residents.

The city says in a statement that investigators have identified several areas of improvement, but they determined officials “did not hide information from the public.”

Mayor Kurt Bradburn says the investigation was necessary for the city to improve.
