CASPER, Wyo. (AP) - An oil well service company faces nearly $3,000 in fines following the death of one its employees who fell from a workover rig in Midwest.

The Casper Star-Tribune reports (https://bit.ly/2dm04sF ) that 28-year-old Dennis McCulloch died in July after falling about 76 feet at the oil well site. State investigators determined he had failed to properly use a safety anchor at the time.

The penalty against McCulloch’s employer, C L Well Service, is lower than the maximum fine of $7,000 for a serious violation because of the company’s small size.

The company couldn’t be reached for comment Wednesday.
