LOS ANGELES (AP) — A report says the Los Angeles Police Department failed to conduct proper welfare checks of inmates at its busiest jail.

The Los Angeles Times reports Saturday that a review by Inspector General Alex Bustamante found 82 percent of 198 checks at the city’s Metropolitan Detention Center were flawed.

The report says jailers in some instances failed to inspect the entire area and in others miscounted the number of inmates.

Jailers are supposed to check each inmate and cell every half hour.

The report was based on 264 hours of security camera footage and paper logs.

Police Commission president Matt Johnson says the findings are troubling but did not appear to cause any inmate injuries or deaths.

He says the department has since taken measures to address the problems.
