PHOENIX (AP) - A hospital group is investigating how its address labels for magazines mailed to some people in Arizona had the Medicare recipients’ identification numbers.

More than 50,000 copies of Banner Health’s 16-page quarterly magazine began arriving Monday in mailboxes in Maricopa and Pinal counties.

The Arizona Republic reports (https://bit.ly/1dvwZ3l) that each recipient’s personal information was printed above the person’s name and address on labels affixed to magazine.

The recipients’ Medicare ID numbers often are identical to their Social Security numbers.

Banner executives say they’re in the process of preparing a letter to the magazine’s recipients to alert them about the matter and to apologize.

Banner is a Phoenix-based non-profit organization that operates 24 hospitals and other related health entities and services in seven states.
